For Payment: We may use and disclose your health information for payment-related functions related to health care services. This may include contacting your insurance provider.
For Health Care Operations: We may use and disclose health information for operational purposes. These uses and disclosures help us improve the quality of care we provide and reduce healthcare costs.
Your Rights Regarding Health Information:
Right to Access: You have the right to access, inspect, and receive a copy of your PHI.
Right to Amend: You have the right to request an amendment to your health record if you believe it is incorrect or incomplete.
Right to an Accounting of Disclosures: You have the right to request a list of the disclosures we have made of your child’s PHI.
Complaints: If you believe your privacy rights have been violated, you may file a complaint with us or with the Secretary of the Department of Health and Human Services. To file a complaint with our office, contact our Office Manager at (541) 686-2446. You will not be penalized for filing a complaint.
